When Can I Start Strength Training After Knee Replacement?

Strength training is essential after knee replacement, but timing and progression matter. Starting too aggressively can harm the scar line and increase swelling; delaying too long can limit recovery.

Early v’s Progressive Strength after Knee Replacement

Early phase rehabilitation is all about movement. The first thing needed is to ensure full knee extension is possible. It is also important to start loading the leg straight away and even doing balance exercises to get the stabiliser muscles working.

Early strength is mostly about:

  • Stabiliser muscles activation.
  • Full Extension
  • Controlled loading

Once the scar is fully healed, strength work can progress, whether on land or in the water, if you have access to a swimming pool. Later stage rehabilitation is about full bending of the knee, correct walking gait and balance work. Do not be afraid to push your new knee, it is very strong and the muscles need to be able to support it properly.

Later stage rehab focuses on:

  • Knee flexion
  • Stability
  • Confidence under demand

Why Structured Progression Matters

It is important to follow the correct progression with a knee replacement. If you push the knee bend too early, for example, you may damage the scar. This can leave you open to infections which will slow your recovery.

However, many people do not push their knee far enough once the scar is healed. Using the static bike to help with full knee flexion and extension in the second stage of knee replacement rehab is important in ensuring strength and movement return.

Using weights in the gym, if you can access one, allows you to load the leg fully. Using machines such as the leg press, leg extension and leg curl machines will allow you to progress your strength.

Walking, squatting and hopping in a swimming pool ( once the scar is fully healed) if a great way to improve your strength and function.

Stability is essential and balance exercises, Bosu exercises and even hopping exercises will help you to get back to full function after your knee replacement.
